Referral Process

 

Early Intervention Services

  • Family contacts Westchester County Department of Health, 914-813-5094, and is assigned to an initial service coordinator, who visits the home and explains the process.

  • Family chooses an evaluator from an approved list and the initial service coordinator schedules the evaluation. Most evaluations occur in the home or community.

  • Completed evaluations are shared with the family.

  • If the evaluation finds that the child is eligible to receive services, the family meets with the team to determine an appropriate plan of service.

  • The ongoing service coordinator chosen by the family assists the family in locating and scheduling service providers to implement the individualized family service plan.

The Children's School for Early Development is approved to provide Early Intervention evaluations, ongoing service coordination, and Early Intervention services under contract with Westchester County.



Pre-School Services

 
  • Family contacts Committee on Pre-School Education (CPSE).

  • Family arranges to complete all necessary evaluations and a meeting with the CPSE follows.

  • CPSE sends referral packet and letter requesting specific programs to The Children's School for Early Development (CSED).
  • The Children's School contacts family to set up an observation.

  • The Children's School contacts parents and CPSE to recommend appropriate sites.

  • Parent contacts site social worker to arrange a tour.

  • CPSE approves placement and Individual Education Plan (IEP) is generated.
