Assistant Habilitation Instructor/Sub Driver - 2 positions - Job# 014-12; 016-12; 015-12 -Yonkers, Peekskill, New Rochelle

Function as a consumer instructor. Assist in developing and implementing community based/in house consumer programming in accordance with their needs, preferences and desires as per their individuals Service & Habilitation Plan. Assist supervisors in overseeing all aspects of the residential program and all direct care staff. Career Supports Coordinator - Job# 066-11 - Hawthorne

Overall responsibility and coordination of consumers, staff and program needs related to community employment in the supported employment department. Acts as a liaison and advocate for individuals with disabilities employed in the community. Maintain contact with employers, families and service coordinators regarding progress or issues at the work site. Responsible for staff paperwork and compliance for billing and audit requirements. Requires HS Diploma, AA or Bachelor’s Degree in Human Services or related field preferred; experience working with DD in rehabilitation preferred, as well as supervision experience; valid NYS Driver’s License and reliable vehicle required. Function as a consumer instructor. Maintain health and safety standards. Position will specifically work within the “Choices” Without Walls program. Staff may be required to operate wheelchair accessible vehicle. The position will support a caseload of individuals with developmental disabilities by providing services such as linkage and referral for benefits and services, advocacy, and monitoring of services and completion of the required corresponding documentation. Flexibility in schedule is necessary. Requires an Associates Degree in a health or human services field, or BA with at least 20 credits in health and human services field, a valid NYS Driver’s License, one year experience working with ID/DD population and basic computer literacy. The ideal candidate should establish and maintain effective working relations, function independently, and be a self-starter.

Parenting Program Assistant - P/T - Job# 009-12 - Yonkers

Primarily assist with the oversight and supervision of toddlers, children and teens of parents who attend the Family Outreach Parenting Program. On occasion may assist the Program Coordinator or Social Worker during the counseling/instructional portion of the Parenting group session. Requires HS Diploma/GED, some college preferred, and some experience in day care, child care or child development work is needed.


Psychologist - Job# 028-12 -Hawthorne Preschool

Develop and monitor comprehensive program for full day children. Administer psychological evaluations. Plan and collaborate with staff for full day preschool classes. Assist social worker in intake process, parents’ visits, family contacts and communication. Provide parent education workshops. Requires Permanent NYS Certification in School Psychology, knowledge of Autism/PPD and principal methods for teaching. Ability to establish and maintain effective working relationship with children, staff and families.


Registered Nurse - Job# 004-12 - White Plains

Provides nursing services to consumers of the residences assigned, assists in the training of other Registered Nurses and department staff, acts as a resource or consultant to Nursing Supervisor and administrations regarding health care issues. Requires graduation of nursing program from an accredited college or university, Bachelors degree preferred; current NYS license as a Professional Registered Nurse; valid NYS driver’s license. Experience working as a nurse in OPWDD approved program preferred.

Carry a pager and be available for crisis and emergency situations 24/7. Requires Bachelors Degree with 1 year experience working with individuals with developmental disabilities or a HS Diploma with at least 3 years experience in residential setting working with individuals with developmental disabilities. A valid NYS driver’s license is required.


Residential Services Director - Job#002-12 - Hawthorne

Oversee the delivery of services to individuals living in assigned residences and assist with ongoing operations of the residential department. Ensure the delivery of person-centered residential services. Ensure that services are provided and documented as per government regulations and laws and agency policy and that billing requirements are met. Supervise residential Program Directors, Clinical Team Leaders and Community Living Coordinators. Be available 24/7 for emergency situations and incident management. Requires a Master's Degree in Psychology, Social Work, Special Education, Public or Health Administration or a related human services field plus at least 3 years of related supervisory or management experience and at least 2 years of clinical experience.

Utilization/Document Review Associate - Job#030-12 - White Plains/ Hawthorne

Assist the Service Coordination and Prevocational Departments in the processing of Medicaid funding documentation and monthly billing. Duties consist in tracking of various records, maintaining logs and forms, preparation of billing records, completion of required reports, and providing administrative support functions. Requires H.S Diploma/GED, a valid NYS Driver’s License, and at least one year experience in performing administrative support functions. Excellent computer skills including Excel are required.
